As a MEDICAL SOCIAL WORKER at Hospice of Warren County, you will:
Assess the emotional, psychological, and social needs of patients and their families.
Collaborate with the interdisciplinary team of doctors, nurses, chaplains, dieticians, counselors to develop a plan of care to address needs related to terminal diagnosis.
Assist patient, family, and staff in utilizing community resources to meet defined needs.
Provide counseling, support, and resources to address emotional and practical issues.
Attend and collaborate with a team of dedicated professionals, including our medical director, nurses, chaplains, bereavement & volunteer coordinators.
Facilitate communication between patients, their families, and the healthcare team.
Maintain detailed records and documentation.
Assess patient/family ability to meet financial obligations and offer appropriate resources and activate as indicated.
Evaluate patient's insurance coverage and options related to hospice benefit.
Assist in discharge planning from hospice service when care is no longer appropriate.
Master's level candidates will provide monthly supervision to non-masters SW staff.
QUALIFCATIONS:
To thrive in this role, you should have:
A Bachelor's or Master's degree in social work.
(Master's level preferred).
Licensure as required by Pennsylvania state regulations.
A compassionate and empathetic personality.
Excellent communication and interpersonal skills.
The ability to work effectively in a team and independently.
